This Enrollment agreement is between  Robert Bartlett

study Alchemy and what you expect to get out of the class or classes.

If ACCEPTED we agrees to provide the
following training, ( please submit separate form for each class):

Course, check one _Prima class , herbal alchemy

                                      _Secunda  (intro to mineral works, pre-req. prima class

                                       _Tertia  (cont. mineral , acetate work, antimony, and animal works)

Each level is $650.00 payable two weeks before class date ($750 for late enrollment)

Primitive Camping available $50 for the weekend (please bring a tent and sleeping bag)

There is usually a potluck meal (barbeque) on Saturday night

Cancellation and refund policy

1.The school will refund all the
money if cancelled within five business days

2.The school will refund all the
money if the student is not accepted.

3.the school may retain a fee of
10 percent of the total tuition cost or
$100 whichever is less. if the student cancels after the 5^th
day of signing the contract or making an initial payment.

Full refund if classes are cancelled or option to apply funds to new date.

The school reserves the right to
cancel a class if the number of students enrolling is insufficient.  In this case the student will be entitled to
a full refund.

This agreement is binding and
must any changes presented in writing.

 LIABILITY RELEASE:

I, (applicant)________________, realize that there are dangers in working with laboratory processes and materials and release Robert Bartlett, all his assigns and heirs from liability from any injury or death caused by working in any laboratory capacity.

Further I realize that this is situated in a natural environment, with livestock, dogs, cats and wild animals as well as uneven ground and will not hold accountable or liable for any injury or death resulting from any animals or environment, Robert Bartlett or his heirs and assigns .

I acknowledge that I will exercise reasonable precautions as far as safety equipment, i.e. gloves and goggles suitable for lab work and prepare for any allergens that may be present to which I am sensitive.
